POSITION SUMMARY: 

 The Nursing Supervisor is a Registered Nurse (RN) who is responsible for the overall supervision of nursing care at High Point Center for Care in both Assisted Living and Memory Care.  He/she will ensure the safety and well-being of all residents; provide direct resident care activities to include assessments, planning, implementing and evaluation of nursing care.  He/she will provide managerial and/or clinical support to residents,  Licensed Practical Nurses (LPN) and Quality of Life Specialists (HHA) to promote good health and achieve high quality of life. 

ESSENTIAL JOB FUNCTIONS: 

  1. Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the resident’s needs.   Demonstrate good decision-making skills.  Provide written evaluation and oral counseling of assigned staff.   
  2. Assist in maintaining charting and documentation on each resident’s status. 
  3. Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, DOH Regulations, and administrative policies. 
  4. 4.Assists with completing vital signs and treatments as ordered by resident’s physician and/or nurse practitioner.
  5. Ensure physicians orders are carried out within a timely manner.
  6. Ensure Narcotic counts are accurate.
  7. Communicate with health care providers to ensure residents have all medication orders and supplies for their care. 
  8. Communicate with resident family members as needed on all issues/concerns for residents.
  9. Participate in the assessment and admission of new residents.  Participate in discharge planning as needed.
  10. Assist as needed with recruiting, hiring, providing orientation/training, and retaining nursing staff.
  11. Responsible for identifying resident medical and behavioral changes and developing and monitoring of residents’ care plan. 
  12. Communicate with interdisciplinary team and Health Services Director on pertinent information regarding resident conditions. 
  13. Assess and monitor complex resident health status and determine the appropriate nursing intervention.
  14. Make rounds with physicians and assist in examinations and/or treatment of residents. 
  15. Perform other duties as assigned.

EDUCATION REQUIRED:

• Must be a graduate from an accredited school of nursing with a current New York State Registered Nurse (RN) license.

• CPR Certification is required.

• Experience with working with Alzheimer's and Dementia residents in an Assisted Living or Memory Care Setting is preferred

• Two (2) years of supervisory/management experience.

KNOWLEDGE/SKILLS: 

• The ability to speak, read and comprehend the English language. 

• Ability to learn and work with the specific needs of those residents suffering from Alzheimer’s disease and/or other related dementias. 

• Willingness to be available for any/all emergencies regarding the facility. 

• Must possess the ability to make independent decisions when situations warrant such action.

• Familiarity with Microsoft Office Suite products and the ability to work with an electronic medical record system.

• Agreement to complete orientation program for Heartfelt Connections Program and other community based training programs.

• Must display patience, tact, enthusiasm, and a cheerful disposition.  

• Must have knowledge of basic nursing care techniques.  Must be willing to learn new techniques and principles and willing to incorporate them into existing resident care practices.

• Must possess excellent communication and customer service skills

• Ability to work as a team and display a positive attitude

 

SPECIAL REQUIREMENTS: 

• Doctor’s statement verifying free from communicable disease 

 PHYSICAL REQUIREMENTS / WORKING CONDITIONS:

• Lift and carry to 100 lbs. occasionally 

• Pushes and pulls up to 100 lbs. occasionally 

• Climbs, reaches, bends and twists occasionally 

• Talks and listens occasionally 

• Sits 2 hours per day; stands and walks 6 hours per day (combined) – 8 hr. day
